Benefits

Lighthouse Placement Services offers three medical insurance options for employees and their families. The Blue Cross Blue Shield HMO provides health coverage for employees living in New England and the PPO provides insurance for employees nationwide. The Aetna plan allows employees to customize their insurance from a range of coverage including medical, vision, term life insurance, hospital indemnity and short term disability. Affordable dental coverage is offered through Altus Dental. Lighthouse Placement Services offers a variety of quality coverage for the health and well being of its employees.

Consultant Benefits

Please note that the following is a summary of benefits. Complete and specific information regarding these benefits may be obtained from the Human Resources Department at Lighthouse Placement Services, LLC.

Direct Deposit

We offer direct deposit after approximately 2-3 weeks of employment.

Pay Period

The workweek begins on Sunday and ends on Saturday. Hours are to be called in/faxed/emailed by 10:00am Monday morning. Weekly paychecks are mailed on Tuesday. A time card with a manager's signature must be received before a paycheck can be released.

Section 125 Cafeteria Plan

This IRS-approved plan allows Lighthouse employees to have weekly medical insurance premiums deducted from their paychecks pretax. Reducing taxable earnings leads to less taxes being taken out of the employees' pay on a weekly basis.

Blue Cross Blue Shield HMO Blue New England $500/$1000 Deductible

This HMO is offered to eligible employees living in New England who have been with Lighthouse for 90 days. Employees must maintain an average of 35 hours per week to become and remain eligible. Covered services include outpatient and inpatient care, prescription drug benefits, mental health and substance abuse treatment as well as a variety of other “Healthy Blue” programs from which to choose. The plan’s deductible is $500 for an individual or $1000 for a family plan. The following services are not subject to the deductible and for the most part cost a copayment of $20: office visits including those for vision and hearing, preventive health services, prescription drugs and all mental health services.
 

Blue Cross Blue Shield HMO Blue New England $1000/2000 Deductible

This HMO is offered to eligible employees living in New England who have been with Lighthouse for 90 days. Employees must maintain an average of 35 hours per week to become and remain eligible. We introduced the Blue Cross Blue Shield HMO Blue New England $1000 Deductible to bridge the cost gap between the Blue Cross Blue Shield HMO Blue New England $500 Deductible and the Blue Cross Blue Shield Preferred Blue PPO Basic $2000 Deductible. This plan has the same excellent coverage as our most expensive plan. It has a lower premium in exchange for a higher deductible. The deductible is $1000 for each member or $2000 per family.


Blue Cross Blue Shield Preferred Blue PPO Basic $2000/$4000 Deductible

Because the PPO network is nationwide, this plan is perfect for Lighthouse employees living outside New England. The PPO is offered to eligible employees who have been with Lighthouse for 90 days. Employees must maintain an average of 35 hours per week to become and remain eligible. Covered services include outpatient and inpatient care, prescription drug benefits, mental health and substance abuse treatment as well as a variety of other “Healthy Blue” programs from which to choose. The plan’s deductible is $2000 for an individual or $4000 for a family plan. The following services are not subject to the deductible and for the most part cost a copayment of $25 in network: office visits including those for vision and hearing, preventive health services and mental health services.
 

Aetna Affordable Health Choices Limited Benefits Insurance Plan

This limited benefits medical plan is offered in most states to employees who have been with Lighthouse for 30 days. For the most part, an employee on this plan pays 20% of the cost for inpatient and outpatient care, physician office visits, preventive visits and prescription drugs. The plan's deductible is $100 for an individual or $200 for a family plan. The maximum benefit per coverage year is $10,000.
 

Vision Care

Although both Blue Cross Blue Shield plans offer one vision exam every 24 months, Lighthouse offers an additional vision care benefit through Aetna.  Aetna Vision Discounts uses the nationwide EyeMed Select Network of vision care providers to offer glasses, contact lenses and LASIK eye surgery at discounted prices. 

Term Life Insurance

Offered through Aetna, the employee term life/accidental death benefit is $20,000.  Optional dependents coverage is also offered.  Benefits are paid to the beneficiary of the employee’s choice. 

Hospital Indemnity

An employee with this Aetna coverage pays 20% of the cost for inpatient care with a deductible of $100 for an individual or $200 for a family plan.  The maximum benefit per coverage year is $10,000. 

Short Term Disability

This Aetna plan offers weekly benefits for up to 6 months while the employee is disabled.  The benefit amount is 50% of the employee’s base pay up to $125 per week. 

Altus Dental’s Altus Plus Dental Plan

Altus Plus is an affordable dental plan offered to employees who have been with Lighthouse for 30 days.  Preventive and diagnostic services are covered 100%; basic/minor restorative services and periodontics are covered 80%; and major restorative services are covered 50%.  The plan’s deductible is $50 for an individual and $150 for a family plan.  The calendar year maximum is $1500. 

Workers' Compensation

An employee may be eligible for state workers’ compensation benefits for job-related injuries. If you are injured while performing a job-related duty, you should immediately report the accident to your supervisor. If your injury results in absence from work, a letter from a doctor stating that you are physically well enough to return to work is required prior to, or on, the date of return.
